Incident Report Form

The Incident Report Form was developed as an initiative to better track incidents within the LMVOA. This form is open to all participants, whether you are a Coach, Player, Referee, Athletic Director, Teacher, or Spectator. We encourage you to submit anything you notice during a match.

Form submissions are anonymous, confidential, and non-punitive. You are not required to submit information which will indicate who you are, however, it would greatly help us if any investigation were to continue. The reports are confidential and only select LMVOA Administrators will assess the situation. Most importantly, the system is non-punitive; there is no punishment or repercussion for errors or mistakes as described on the report.
